About the role
We are seeking compassionate Licensed Practical Nurses (LPNs) to provide high-quality, patient-centered care in patients' homes within the Green Bay area.
Responsibilities
- Deliver skilled nursing care following physician orders and care plans
- Assess, monitor, and document patients' physical and emotional health
- Identify condition changes and communicate findings effectively
- Develop and implement individualized care interventions
- Collaborate with patients, families, and healthcare teams to ensure quality care
- Maintain accurate clinical documentation
Qualifications and Skills
- Current, unrestricted LPN license in Wisconsin
- Current BLS/CPR certification
- Recent TB test or Chest X-Ray
- Preferably one or more years of nursing experience
- Strong clinical, communication, and documentation skills
- Dedication to compassionate, patient-focused care
