About the role
Provide skilled nursing services in patients' homes under supervision of a Registered Nurse.
Responsibilities
- Deliver patient-centered care and education in a home setting
- Assess, plan, implement, and evaluate patient needs and care to promote recovery and manage chronic conditions
- Maintain accurate documentation and medication updates
- Monitor and respond to changes in clinical status
- Communicate effectively with patients, families, and healthcare professionals
Qualifications
- Graduate of an accredited practical nursing program and current LPN licensure
- Minimum of 1 year of experience
- CPR certification; LPN-IV certification or coursework within 3 months if applicable
- Must be a licensed driver and meet state licensing requirements
