About the role
The Licensed Practical Nurse (LPN) in Home Health provides quality patient care in patients' homes, performing skilled clinical services within the scope of practice defined by state laws.
Responsibilities
- Thoroughly document patient visits in electronic medical records
- Participate in care coordination by reporting patient needs and changes
- Attend case conferences
- Communicate effectively with patients, families, physicians, and care team members
- Provide compassionate, equitable care and adhere to all applicable regulations and standards
Requirements
- Current, unrestricted LPN licensure
- CPR certification
- Valid driver’s license
- Dependable transportation
- Ability to work flexible hours

Pay
