About the role

Provides quality patient care in patients' homes, performing skilled clinical services within their scope of practice and following the care plan. Documents visits thoroughly in electronic medical records, participates in care coordination by reporting patient needs and changes, and communicates effectively with patients, families, physicians, and team members.

Requirements

• Current and unrestricted LPN licensure.
• CPR certification.
• Valid driver’s license or access to reliable transportation.
• Flexibility to meet patient needs.

Pay

Compensation ranges from $59,498 to $89,247 annually, based on productivity and role activity, including per visit and hourly pay.
