About the role

Provide quality patient care in clients' homes within the scope of practice defined by state laws, following the plan of care and collaborating with the healthcare team.

Responsibilities

• Instruct family members on client care and maintaining a healthy environment.
• Observe, document, and report patient responses and condition changes to supervising nurses or physicians daily.
• Function effectively in diverse home settings, regardless of age, race, creed, or financial status.
• Deliver skilled nursing services and patient education.
• Monitor and document patient progress and condition.
• Collaborate with healthcare team members to ensure quality care.

Requirements

• Valid and unrestricted LPN license in the practice state.
• Reliable transportation, such as a vehicle or access to public transit, and a current driver’s license.
• Ability to work flexibly to meet patient needs.

Preferred Qualifications

• CPR certification.
• Home care experience.
• Independence, strong communication, organizational, and writing skills.
