LPN - Home Care

Responsibilities

• Provide in-home patient care following the prescribed plan of care and nursing scope of practice
• Assess and document patient conditions, monitor progress, and report changes
• Administer medications, insulin, IV fluids, and document thoroughly
• Inspect and care for wounds, change dressings, and assist with personal hygiene
• Evaluate patient for injuries, infections, bedsores, and deliver appropriate treatments
• Collaborate with healthcare team to ensure optimal patient outcomes
• Deliver direct patient care with compassion and professionalism
• Monitor and document patient status accurately
• Coordinate with healthcare professionals to ensure quality care

Qualifications & Skills

• Graduate of an accredited Nursing program
• Current unrestricted LPN license in the relevant state
• CPR/AED/BLS/First Aid certification
• Reliable transportation to care sites
• Practical trach and/or ventilator experience preferred but not required
