Responsibilities
- Provide in-home patient care according to prescribed plans, monitoring progress and recovery
- Assess, document, and report patient conditions, including vital signs and symptoms
- Administer medications, insulin, and IV fluids with thorough documentation
- Inspect and care for wounds, change dressings, and assist with personal hygiene
- Evaluate for injuries, infections, or bedsores and deliver appropriate treatments
- Collaborate with healthcare providers and team members to ensure quality patient outcomes
Qualifications & Skills
- Graduate of an accredited nursing program
- Current unrestricted LPN license in the state of practice
- Current CPR, AED, BLS, and First Aid certifications
- Reliable transportation to care sites
- Optional but preferred: practical trach and/or ventilator experience

- Deliver personalized, direct patient care in-home setting
- Monitor and document patient health status and progress
- Administer medications and treatments accurately
- Maintain wound care and hygiene standards
- Collaborate with healthcare team to optimize patient wellbeing
