Role Overview Assist in delivering high-quality nursing care under the supervision of a registered nurse in a home health setting. Responsibilities Include Providing safe, effective, and documented care to clients of all ages, following established plans of care.Conducting client assessments, implementing nursing interventions, and communicating changes to the RN.Documenting visits accurately, ensuring compliance with policies and regulations.Coordinating care by communicating with clients, caregivers, and healthcare team members.Performing lab tests, recognizing client needs, and referring to other disciplines as necessary.Maintaining punctuality, flexibility, and efficient use of resources to maximize client care.Engaging in ongoing professional development and staying current with healthcare trends and certifications. Qualifications And Skills Completion of an LPN program with at least one year of experience.Valid Illinois LPN license, proof of auto insurance, BLS certification, and a valid driver's license.Strong communication, documentation, and organizational skills.Ability to provide skilled care for diverse patient populations and adapt to changing needs.
