About the role
Licensed Practical Nurse (LPN) providing in-home patient care.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services according to prescribed orders.
- Monitor signs and symptoms to identify physiological and psychosocial changes.
- Collect, analyze, and document data to determine nursing diagnoses.
Qualifications
- Current LPN/LVN license in the practicing state.
- Valid TB test or Chest X-Ray and BLS certification.
- Preferred: at least one year of professional nursing experience.
