Position Summary

Provides primary care patients in direct collaboration with the provider (NP and/or MD). Must be flexible to the needs of Hamilton Center including working in other areas of the organization. Functions as a part of the interdisciplinary treatment team under the direction of the Nurse Practitioner(s) or Medical Doctor.

Essential Duties/Responsibilities

  • Performs duties under the direction of a nurse practitioner or medical doctor.
  • Administers medications / vaccinations / inoculations following established procedures and Standards of Care.
  • Participates as a member of the interdisciplinary team by carrying out treatment interventions within the limits of training and licensure, and by communicating observations regarding patients to other members of the treatment team.
  • Documents all care provided and patient response in the patient's medical record per established procedures and Standards of Care.
  • Conducts patient/family education related to medications, and general physical and mental health needs.
  • Participates in Quality Improvement activities as assigned.
  • Maintains professional behavior consistent with the principles of customer relations, teamwork, and federal guidelines and expectations.
  • Maintains confidentiality in accordance with established HIPAA policy and procedure.
  • Participates in staff meetings and continuing education activities to continually update current knowledge regarding nursing in general.
  • Flexibility in work schedule as determined by organizational needs.
  • Maintains basic knowledge of computer software utilized for patient recordkeeping.
  • Performs other duties as assigned.

Minimum Qualifications/Requirements

  • Knowledge and skills regarding basic nursing care and general knowledge regarding preparation and administration of vaccinations / inoculations.
  • Good oral and written communication skills.
  • Basic computer skills.

Certificates, Licenses, Registrations

  • Licensed by the State of Indiana as a Licensed Practical Nurse.
  • Maintain current American Red Cross CPR/AED certifications.
  • Maintain current Crisis Prevention Intervention (CPI) certification.
  • Valid Drivers License in accordance with HCI motor vehicle policy.
  • Completes all required continuing education requirements in a timely manner.

PHYSICAL DEMANDS

  • Exposure to patients at various levels of mental / emotional stability is possible.
  • Physical strength necessary to perform the essential functions.
  • Sitting, standing, reaching, bending, walking, stooping as necessary to provide individualized patient care.

Work Environment

  • Requires corrected vision and hearing to normal range.
  • While performing the duties of this job, the employee is regularly required to talk or hear.
  • The employee regularly is required to stand, walk, and sit.
  • Work is performed in an office environment.
  • Sometimes bends and stretches required.
  • Use of telephone is required.
  • Manual dexterity required for use computer keyboard.
  • Occasionally lifts and carries items weighing up to 25 pounds.
  • Occasionally pushes patients in wheelchairs.

Conditions of Employment

  • Completion of tuberculin screening no later than three days prior to first day of employment and annually thereafter.
  • Completion of MMR, Varicella, influenza and coronavirus vaccine.
  • Completion of pre-employment drug screening and completion of post-employment drug or alcohol tests upon reasonable suspicion of use.
  • Completion of Center-wide orientation and ALL required paperwork prior to reporting for work.
  • Demonstrated computer literacy through successful completion of pre-employment testing may be required.
  • Completion of HCI Quality training, Trauma-Informed Care, and Relias Learning.
  • Attendance at all mandatory staff development and training.
  • Successful completion of a six-month on-the-job orientation period.
  • Participation in payroll electronic deposit.
  • Adherence to Compliance Program Plan.
  • Satisfactory reference and background investigation checks.
  • OBHP qualified staff will be expected to bill MRO services, document those services in the electronic medical record and to participate in required clinical supervision.
  • OBHP qualified staff will be required to complete OBHP modules within 90-days of hire.
