About the Role
Responsible for providing nursing care to clients on assigned area. Supervises and directs subordinate nursing personnel (e.g., nursing assistants) per RN directive. Assists the RN in a variety of client care activities and completes delegated work-related tasks as assigned.
Responsibilities
- Administers and documents medications and treatments on the electronic MAR as prescribed by physicians and according to established nursing policy/procedure.
- Makes rounds with physician and completes order entry in Avatar under supervision of the Registered Nurse.
- Maintains current information on new medications, dosages, and side effects.
- Provides medication teaching with client/family as indicated in client care plan and as directed by the Registered Nurse.
- Provides nursing care to meet the physical, emotional, and psycho-social needs of the client.
- Participates in the development and implementation of client care plans.
- Observes and documents changes in client behavior/condition and informs RN accordingly.
- Participates in emergency or crisis situations as directed by the Registered Nurse.
- Assists with admissions, discharges, and transfer of clients.
- Maintains records, reports, and documentation in accordance with departmental standards.
- Maintains a clean, comfortable, and safe environment for clients.
- Assists in creating a therapeutic atmosphere on the client unit.
- Maintains effective working relationships with other employees.
- Attends and participates in staff meetings, mandatory in-services, and other educational programs.
Requirements
- Knowledge and understanding of the terminology, techniques, and practices of Practical Nursing.
- Knowledge of basic needs of geriatric, developmentally disabled, mentally ill, AODA, and child/adolescent clients.
- Knowledge of basic pharmacology and specifically psychopharmacology (expected actions, side effects, contraindications, and route of administration, including types of injectables).
- Knowledge of principles of documentation.
- Knowledge of basic principles of behavior modification and appropriate application of behavior modification techniques.
- Ability to complete basic Medicare evaluations following proper training and competency demonstration.
- Ability to recognize and address the physical, psycho-social, mental, and emotional needs of clients.
- Ability to establish and maintain therapeutic relationships with clients and their family members.
- Ability to carry out a nursing care plan as directed by the Registered Nurse.
- Ability to maintain accurate records and enter physician orders into the electronic record system under RN supervision.
- Ability to use all standard medical equipment.
- Ability to perform in emergency situations as directed by the Registered Nurse.
- Ability to recognize and maintain confidentiality of all matters related to client care.
- Ability to understand and carry out policies and procedures governing client care.
- Ability to participate in unit activities and serve as a role model for clients.
- Ability to function within the organizational structure utilizing the "chain of command."
- Ability to follow oral and written instructions.
- Ability to establish and maintain effective interpersonal working relationships.
- Ability to communicate effectively both orally and in writing.
- Must be free from communicable disease.
Physical Demands
- Lifting 75 pounds maximum with assistance; frequent lifting/carrying of objects up to 40 pounds.
- Extended periods of standing, walking, and sitting; occasional driving.
- Using hands for repetitive single grasping, fine manipulation, pushing, pulling, and operating controls.
- Frequent bending, twisting, squatting, climbing, reaching, and grappling; occasional swimming.
- Communicating orally in a clear manner.
- Distinguishing sounds at various frequencies and volumes.
- Distinguishing people or objects at varied distances under a variety of light conditions.
Schedule
- Full-time (AM and PM shift), part-time, and on-call positions available.
- All part-time and full-time nursing positions work every other weekend.
- Additional pay incentives for working PM shift, NOC shift, and weekends.
- Additional special assignment pay for .2, .4, and on-call positions.
Pay
On-Call LPN base pay rate: $30.33 + additional $5 special assignment pay.
