Responsibilities
- Return or make follow up phone calls to patients as directed by the Triage RN
- Under the guidance of the Triage RN, makes appropriate recommendations related to assessment in accordance with TL’s policy and procedures.
- Under the guidance of the Triage RN, communicates with attending physician or TL on-call physician regarding any changes in patient’s status and/or needs for medication and/or DMEs.
- Under the guidance of the Triage RN, instructs patient and/or primary caregiver on any changes in patient’s Plan of Care.
- Assigns visits with the Triage RN to RN, SW, or chaplain when indicated.
- Under the guidance of the Triage RN, assists in making arrangements for transfer to inpatient facilities which may include obtaining orders, assessing bed availability, and communicating with facility staff.
- Contacts DME companies, pharmacies, and transportation services as needed.
- Under the guidance of the Triage RN, takes weekend referrals and arranges for admission or information visits as needed.
- Makes prn visits to patients as directed by the Triage RN.
- Communicates with patient’s primary team members and on call staff concerning any calls received.
- Communicates with management on call if indicated.
- Documents in patient medical records to meet federal, state, regulatory and agency guidelines.
- Aids in obtaining information for Hospice medical records including but not limited to lab reports, history and physicals and progress notes.
- Conducts chart audits, QA\UR, specified studies, and assists in the office as assigned.
- Maintains HIPAA privacy standards.
- Attends seminars/workshops as appropriate.
- Performs other duties as assigned by supervisor.
Requirements
- Licensed Practical Nurse with current N.C. license
- Minimum 2 years recent clinical experience
- Knowledgeable about reimbursement sources, Medicare regulations, licensing laws, and accreditation standards
- Competent computer skills
- Current CPR certification
- Must have and maintain a valid North Carolina Driver’s License, maintain automobile insurance coverage and have access to reliable transportation
- Must be able to travel to, enter and function in the homes/facility of any patient/family in the service area to assess and provide care to all patients
- May be required to lift and/or carry items up to 50 pounds
- Must have sensory abilities to complete physical assessment, gather data regarding patient conditions, communicate with patient/family care providers and physician and to complete the reporting and follow-up documentation responsibilities of the job
- Must be available to work on or around holidays as scheduled and potentially on a rotating basis
