About the role
Licensed Practical Nurse (LPN) providing skilled nursing care in various facilities within Hampton Roads.
Responsibilities
- Administer medications and treatments within the LPN scope of practice
- Collect, document, and monitor patient information and condition changes
- Perform wound care and provide patient education
- Assist with activities of daily living as needed
- Coordinate care with healthcare team members
- Perform additional duties based on facility requirements
Qualifications & Skills
- Active LPN license
- Minimum of 1 year of facility-based experience
- Current BLS/CPR certification
- Recent PPD within the last year
- Ability to meet the physical demands of the role
