LPN Float (LPN-IV) - Little Rock Markert

About the role

The LPN Float will be based in the Little Rock Market location. Will train new clinical staff. Will triage patient calls for the center in order to prioritize medical care based on the severity of the patient’s condition. Additionally, will provide direct patient care, ongoing education and reinforcement of care plans, and providing general support for the patient under the physician's direction. Includes preparing patients for exams, assisting during treatments, coordinating tests and follow-up care, and recording treatment in their electronic health record. Assists with explaining patient instructions to the patient/family. Demonstrates competency and skills to serve patients in geriatric populations.

The LPN Float displays an elevated level of professionalism and engages resources and fellow team members within the center to deliver high level results.

Primary Responsibilities

• Responsible for the daily activities of the clinical staff.
• Aid the provider through data collection concerning the physical, psychological, social, and cultural dimensions of patients according to practice standards and policy/procedure.
• Implement age-appropriate interventions based on individual patient’s needs as directed by the provider.
• Aid in evaluating the effectiveness of care given in progressing patients toward desired outcomes.
• Provide ongoing education and reinforcement of care plans developed by the Provider based on identified health and education needs and the condition and age of the patient.
• Display teamwork in the delivery of patient care.
• Aid the Provider with triaging of patient portal messages.
• Triage phone messages and provide feedback and answers to patient/physician/pharmacy calls.
• Make calls to pharmacy, diagnostic facilities, etc. as required by the Provider.
• Perform basic wound care.
• Perform suture/staple removal and dressing changes as indicated.
• Medication reconciliation at point of care.
• Prepare prior prescription authorization requests for provider to review and finalize.
• Manage/monitor providers clinical inbox in the EHR and triage as necessary per the direction of the provider.
• Aid in preparation of clinical components for Provider review and finalization.
• Prepare medication refills for Provider review and finalization.
• Assist with blood draws and IV insertions for fluid infusion under the supervision of the Provider.
• Participate in center interdisciplinary team meetings related to patient care plans and outcomes.
• Ensure the overall quality of healthcare for each patient.
• Ensure patient electronic health records are updated with the most recent medical treatments and records from outside facilities.
• Under the leadership of the Market Clinical Nurse Manager, will train and orient center clinical staff, i.e. Medical Assistants.
• Manage the flow of patients being seen and the performance of the clinical staff.
• Assure quality gaps are being addressed.

Qualifications

• Graduated from an accredited Licensed Practical Nurse Program.
• Licensed to practice as a Practical Nurse (LPN) in Arkansas.
• Minimum Nursing Experience: 5 years
• Minimum Management Experience: 3 years
• Minimum Certifications and/or Licenses: CPR and first aid certification required.
• Experience with EMR.
• Phlebotomy (blood draw)

• Active Listening is essential.
• IV Therapy Certification is required.
• Willing to travel between center locations as needed.
