About the role
Please Note: New Directions is the Geriatric Behavioral Health Unit for Dodge County Hospital.
Responsibilities
- Provides direct and indirect patient care services that meet the psychosocial, physical and general aspects of care;
- Maintains and operates equipment safely and correctly;
- Treats patients and families with respect and dignity;
- Communicates with physicians and team members about changes in patients' conditions, including results of diagnostic studies and symptomatology;
- Communicates appropriately and clearly to team leader, coworkers, and physicians;
- Consults other departments, as appropriate, to provide for and interdisciplinary approach to the patient's needs;
- Coordinates and supervises patient care under the direct supervision of a Registered Nurse;
- Assists with research studies conducted in the department;
- Communicates with physicians and team members about changes in patients' conditions, including results of diagnostic studies and symptomatology;
- Performs patient care responsibilities considering needs specific to the standard of care for the geriatric patient and the patient's clinical condition;
- Demonstrates the ability, under the direct supervision of a RN, to perform nursing assessments on all patients and reassessments as per policy;
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors;
- Demonstrates knowledge of medications including psychotropic drugs and their correct administration based on the patient's clinical condition;
- Follows the five medication rights and reduces the potential for medication errors;
- Demonstrates ability to revise the plan of care as indicated by the patient's response to treatment and evaluate overall plan daily for effectiveness;
- Formulates and implements patient treatment plans; family is included when appropriate;
- Demonstrates ability to perform treatments and provide services to level of licensure;
- Maintains and operates equipment safely and correctly;
- Communicates appropriately and clearly to team leader, coworkers, and physicians;
- Concurs with members of the treatment team for the benefit of the patient;
- Provides nursing care appropriate to the mental and physical condition of the geriatric patient;
- Documentation meets current standards and policies;
- Provides education to the patient and family concerning treatment plans, compliance with the treatment plan, medications, and their side effects within scope of practice;
- Communicates with physicians and team members about changes in patients' conditions, including results of diagnostic studies and symptomatology;
- Performs patient care responsibilities considering needs specific to the standard of care for the geriatric patient and the patient's clinical condition;
- Demonstrates an ability to be flexible, organized and function under stressful situations.
Requirements
- BLS Certification (Preferred)
- LPN LICENSE (Required)
Qualifications
- Proven experience as LPN or similar role;
- Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team;
- Familiarity with hospital health, safety and sanitation standards and procedures;
- Understanding of confidentiality obligations and nursing best practices;
- Excellent knowledge of medical and hospital terminology;
- Good knowledge of MS Office and data entry;
- Excellent communication and interpersonal skills;
- Caregiver must be compassionate and able to handle stress.
Skills
