Position Summary
The CCBHC Mobile Crisis LPN will be part of a 24-hour crisis stabilization hub that includes enhanced 24-hour crisis services and a mobile crisis unit providing crisis stabilization services to the identified populations. Functions as a part of the interdisciplinary treatment team, in which patient care is prescribed.
Essential Duties/Responsibilities
- Demonstrates the ability to conduct and monitor the nursing process, which consists of assessment, planning, implementation and evaluation of nursing care.
- Conduct admission assessments and reassessments as indicated according to policy, procedures and standards of care.
- Follow admission and discharge procedures.
- Works closely with law enforcement and community agencies to provide immediate intervention aimed at reducing the risk of emergency room and psychiatric hospitalization.
- Functions as a member of The Mobile Crisis team consisting of Crisis Diversion Specialists, on-call therapists and medical team.
- Afford assistance in connecting the individual to appropriate level of care.
- Obtain and transcribe correctly the licensed independent practitioners (LIP) orders for admission to the inpatient unit.
- Participate as a member of the interdisciplinary team.
- Document in the patient's record according to policy and procedure.
- Afford assistance in the identification of patient and family educational needs.
- Provide daily information at shift change and treatment review/discharge meetings.
- Participate in CQI activities as assigned.
- Complete safety plans and internal referrals for continued services after crisis has been stabilized.
- Implement crisis intervention techniques when dealing with emergency situations.
- Provides crisis assessments under the supervision of the on-call psychiatrist.
- Gather clinical information and staff with on-call psychiatrist.
- Serve as a resource to Hamilton Center staff and community regarding available services, referral sources, emergency detention procedures, etc.
- Participate in cross-training as assigned.
- Ability to demonstrate competent use of Electronic Medical Record.
- Afford assistance in answering inquiring phone calls, respond to requests for information/referral sources and route calls as appropriate.
- Review referrals from outside agencies for HCIs inpatient unit and staff accordingly with on-call providers orders.
- Maintain a high level of ethical conduct regarding confidentiality, dual-relationships and professional stature.
- Keep supervisor informed of potential crisis situations.
- Maintains basic computer skills and knowledge in order to access client information and/or update information.
Minimum Qualifications/Requirements
- Knowledge and skills regarding basic nursing care and general knowledge regarding preparation and administration of medications.
- Good oral and written communication skills.
- Basic computer skills
Certificates, Licenses, Registrations
- Licensed by the State of Indiana as a Licensed Practical Nurse.
- Maintain current crisis prevention on/intervention and American Red Cross CPR/AED certification.
- Maintain current Crisis Prevention Intervention (CPI) certification
Physical Demands
- Exposure to disturbed and potentially aggressive patients is possible.
- Stress ranges from low to intense depending on situation.
- Physical strength necessary to implement CPI interventions.
- Sitting, standing, reaching, bending, stooping as necessary to provide individualized patient care.
Work Environment
Work is normally performed in a comfortable physical environment within a scheduled daily regimen with steady time pressure, multifaceted tasks, and ongoing interruptions.
Conditions of Employment
- Completion of tuberculin screening no later than three days prior to first day of employment and annually thereafter
- Completion of MMR, Varicella, influenza and coronavirus vaccine
- Completion of a pre-employment drug screening and completion of post-employment drug or alcohol tests upon reasonable suspicion of use.
- Completion of Center-wide orientation and ALL required paperwork prior to reporting for work
- Demonstrated computer literacy through successful completion of pre-employment testing may be required.
- Completion of HCI Commitment to Quality training, Trauma Informed Care, and Relias Learning.
- Completion of OBHP modules within the first 90-days.
- Attendance at all mandatory staff development and training
- Successful completion of a six month on the job orientation period
- Participation in payroll electronic deposit
- Adherence to Compliance Program Plan
- Satisfactory reference and background investigation checks.
