About the role
West River Care is a family-run organization offering comprehensive home health care services in the Northeast. We serve people who wish to live independently, manage their health, or receive hospice care. Our mission is to support individuals and families in their homes.
Responsibilities
- Communicates changes in condition to RN case manager and/or physician
- Participates in care coordination meetings
- Arranges and orders equipment, supplies, and medications related to patient care
- Prepares clinical notes and updates the primary physician when necessary
- Communicates with the interdisciplinary team and the physician regarding the patient's needs and changes in condition
Requirements
- License to practice as an LPN under the direction of the RN case manager
- One year of hands-on clinical experiences as a nurse (in any field of nursing)
- Hospice experience preferred but not required
- Current license in state and CPR certification
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist
- Prolonged or considerable walking or standing
- Able to lift, position or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling or crouching
- Visual acuity and hearing to perform required nursing skills
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Schedule
Full-Time, 32-40 hours per week -- guaranteed hours!
