Responsibilities
- Provide patient care under supervision, ensuring safety and comfort for residents.
- Aid with medications and routine treatments according to physician orders and policies.
- Maintain close monitoring of patient status and promptly escalate concerns to supervising nurse.
- Ensure strict adherence to safety, infection control, and quality standards.
- Document care plans, progress notes, and vitals in electronic health records.
- Collaborate with interdisciplinary team to support patient-centered goals.
- Maintain high productivity with measurable results and meet performance targets.
- Offer compassionate, patient-centered care while respecting privacy and dignity.
- Support continuous learning through training, certifications, and on-the-job development.
- Assist with patient admissions, discharges, and routine transportation within the facility.
- Maintain equipment readiness and report malfunctions promptly.
Requirements
- Licensed Practical Nurse (LPN) license valid in the applicable state.
- Experience delivering direct patient care in long-term or facility settings.
- Proficiency with electronic health records and accurate documentation.
- Ability to follow physician orders, administer medications, and monitor treatments.
- Strong infection control, safety, and patient privacy practices.
- Excellent communication and collaboration with interdisciplinary teams.
- Commitment to high productivity, meeting performance targets, and measurable outcomes.
- Compassionate, patient-centered care with respectful handling of dignity and privacy.
- Willingness to participate in ongoing training, certifications, and professional development.
- Ability to assist with admissions, discharges, and routine facility transport.
