Responsibilities

- Demonstrate reliability in a results-driven environment with transparent performance metrics.

- Provide compassionate patient and family education on treatments and care plans.
Qualifications
- Active LPN license in the applicable state or region.
- Prior patient care experience in acute or ambulatory settings.
- Ability to document observations and outcomes accurately and promptly.
- Medication administration competency following safety protocols.
- Strong understanding of privacy laws and healthcare compliance standards.
- Proven ability to work under supervision within a multidisciplinary team.
- Demonstrated reliability in a metrics-driven, target-focused environment.
