Basic Function
Incumbent is responsible for providing professional nursing care under direction of staff RNs for patients receiving medical care following established best practices and standards.
General Responsibilities
- Works with the assigned care team to ensure patient health maintenance requirements are accurate, up to date, and documented appropriately.
- Proactively reviews schedule to meet caseload requirements, keep abreast of updates, and inform case managers and providers of any changes.
- Prepares patients for, and assists providers with, examinations and procedures related to direct patient care.
- Performs any/all nursing duties (within licensure) required to provide high quality patient care.
- Obtains pertinent clinical data from patients. Records information accurately and in a timely manner into the electronic health record.
- Maintains a safe, comfortable, and therapeutic environment for patients and families in accordance with SNHS standards.
- Provides culturally sensitive care for patients of all ages.
- Responsible to handle appropriately any/all communications necessary for patient care, both incoming and outgoing.
- Documents all patient contacts according to regulations and policies.
- Generates reports as needed.
- Responsible for all general maintenance, housekeeping, material stocking, and cleaning as needed in order to maintain a clean and sanitary workplace for the delivery of patient care.
- Promotes a cooperative and cohesive workplace environment to maintain high quality services to patients.
- Participates and promotes PCMH guidelines and procedures.
- Responsible for any/all electronic health record functions and duties required for patient care.
- Enhances professional growth and development through participation in educational programs, review of current literature, and various workshops as available.
- Upholds and promotes the value and integrity of the Seneca Nation Health System’s mission statement. Will promote SNHS in a positive manner.
- Provides care through a trauma-informed lens by incorporating key principles of both physical and psychological environment safety, trustworthiness and transparency, collaboration of decision-making during treatment plan developments, empowerment of patients by recognizing and building upon individual strengths, as well as recognizing and respecting the diversity of cultural backgrounds and sensitivity.
Qualifications
- Current Licensed Practical Nurse NYS licensure required.
- Minimum of 2 years clinical experience required.
- Current CPR/BLS required.
- Experience in a large hospital, academic medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.
