Job Summary
As a LPN you will play a pivotal role in managing patient care to ensure the highest standards are met.
Responsibilities
- Assessment and Care Planning: Conduct ongoing assessments of patients' health status. Implement individualized care plans based on these assessments to meet the patient's needs.
- Medication Administration: Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects.
- Wound Care: Provide wound care management, including dressing changes, infection control, and monitoring healing progress.
- Patient Education: Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self-care and management of their health at home.
- Monitoring and Evaluation: Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes.
- Collaboration: Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and integrated care for the patient.
- Documentation: Maintain accurate and up-to-date medical records and documentation of nursing assessments, interventions, and patient responses.
- Support and Advocacy: Provide emotional support and advocacy for patients and their families. Ensure patients' rights and preferences are respected throughout their care.
Qualifications
- Licensed Practical Nurse (LPN) with current licensure in Indiana
- Bachelor’s degree in nursing
- Previous experience in home care
- Strong leadership and communication skills
- Proficient in Microsoft Office Excel, Microsoft Word and electronic medical records systems
