Job Overview
Title: LVN/LPN- Non-Exempt
Duties and Responsibilities:
- Provides skilled nursing as ordered on the plan of treatment and nursing care plan.
- Identifies new problems, changes, and/or resolution or problems and follows through as directed.
- Notifies Director/Supervisor and Physician as needed.
- Documents all actions in clinical notes, supplemental physical orders, and nursing care plan.
- Performs aseptic techniques as required.
- Administers specific treatments and medications as per care plan.
- Works as a team member under Registered Nurse’s direction.
- Communicates skilled observations and significant changes in patient status to appropriate personnel.
- Requests modifications to orders or additional orders based on clinical status and judgment.
- Documents all physician collaboration in the medical record.
- Adheres to agency policies, procedures, and handbook.
- Completes clinical documentation accurately and submits it on time.
- Behaves professionally in the company and community.
- Reports on time and completes work within designated time.
Requirements
- Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post graduate education.
- Valid/unencumbered verification/license to practice the job as described in the state where services are provided.
- Professional experience as required per state regulations.
- Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater.
- Reliable transportation.
- Current TB results or Chest x-ray.
- CPR certification as per agency policy.
- Flexibility, organization, and ability to exercise independent judgment.
Physical Requirements
- Sit, stand, bend, kneel, walk, crouch, crawl, push, pull, and reach.
- Ability to lift and/or move up to 15 pounds and occasionally lift or move 100 pounds or more.
- Lighting, acoustics, air quality, temperature, and environment hazards are not predictable.
- Exposure to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
- Required to travel frequently for home health visits.
