About the role
This position is responsible for providing safe therapeutic care in a holistic and systematic way. The LPN will incorporate differences into the provision of care, integrate knowledge, skills and experiences to meet the needs of the patients and families throughout the continuum of care. This position will be responsible for providing oversight and supervision to the LNA's and communicating with healthcare team members and participating in multidisciplinary meetings.
Responsibilities
- Provide safe therapeutic care in a holistic and systematic way
- Incorporate differences into the provision of care
- Integrate knowledge, skills and experiences to meet the needs of the patients and families throughout the continuum of care
- Provide oversight and supervision to the LNA's
- Communicate with healthcare team members and participate in multidisciplinary meetings
- Perform routine physical/psychosocial assessments to collect basic data
- Manage medications and chart in a timely manner and within policy guidelines
Requirements
- Vermont LPN licensure
- BLS certification (current/renewed every 2 years)
