LPN
Job Summary
The LPN must be clinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members.
Compensation
- LPN: $28.00 – $32.83 per hour dependent on experience PLUS $2.00 per hour PM and $2.50 per hour NOC shift differentials
- LPN Weekend Warrior: $28.00 – $32.83 per hour dependent on experience PLUS shift differentials - $2.00 per hour PM shift and $2.50 per hour NOC shift

Key Responsibilities
- Provide safe, effective nursing care per care plans and physician orders
- Administer medications and treatments accurately
- Assess residents and report changes promptly
- Document care in the EMR
- Monitor performance and mentor CNAs
- Ensure infection control and safety compliance
- Completes all other duties as assigned.
Qualifications
- Required: Active LPN license in state of Wisconsin or licensed in any U.S state and be eligible for Wisconsin licensure by reciprocity. (in good standing)
- CPR certified
- Ability to meet physical and job-related requirements
- Must be English literate and possess excellent written and verbal communication skills
