About the role

The LPN is responsible for providing quality patient-centered, evidenced-based care to our residents. Duties include initial data collection, implementing person-driven care plans, and utilizing principles of Relationship Based Care.

Responsibilities

• Initial data collection on all residents
• Implementing person-driven care plans as developed by RN
• Utilizing principles of Relationship Based Care

Requirements

• Current licensure as a Practical Nurse in good standing
• Ability to read, write, and communicate in English
• Clinical experience preferred
