About the role
This per diem Licensed Practical Nurse (LPN) position at Breton Assisted Living & Health Centre offers a unique opportunity to support both Breton Extended Care Assisted Living (BEC) and the Breton Woods Health Centre. The role provides a rewarding blend of clinical care, resident advocacy, and collaboration across multiple levels of care on the beautiful Breton Woods Campus in Grand Rapids, Michigan.
Responsibilities
- Provide direct LPN care, including medication administration, treatments, assessments, vital signs, documentation, and timely communication of condition changes to physicians.
- Support daily clinical operations by assisting with staffing coverage, orienting new staff, and coaching team members under nursing leadership guidance.
- Monitor resident care needs in Assisted Living and Independent Living settings to promote quality care and regulatory compliance.
- Serve as a liaison among residents, families, physicians, and the interdisciplinary team to coordinate care and support admissions.
- Respond to resident emergencies and assist with physician procedures while delivering compassionate, resident-centered care aligned with organizational values.
- Provide nursing care to Independent Living residents who utilize Health Centre services on the Breton Woods Campus.
- Build and maintain trusting relationships with residents and families while addressing concerns with professionalism and empathy.
- Coordinate scheduling and appointments with physicians.
- Assist with transfers to higher levels of care, ensuring timely and accurate documentation.
- Collaborate with Independent Living leadership and partners across Assisted Living, Skilled Nursing, Faith Hospice, and Atrio Home Care.
Requirements
- Graduate of an accredited LPN program with current Michigan LPN licensure.
- Minimum of 2 years of nursing experience.
- Previous experience in emergency room or home care case management strongly preferred.
- Demonstrated strong assessment skills and ability to make independent clinical decisions.
