LPN - Assisted Living

About the role
We are seeking an experienced Licensed Practical Nurse (LPN) to join our client, a leader in home health care, in one of their Assisted Living Facilities. In this role, the LPN will perform nursing tasks and responsibilities under the direction of a Registered Nurse (RN), providing compassionate, high-quality care to residents. CHHA experience is preferred. This is an excellent opportunity for a nurse looking to take the next step in their professional journey while working in a supportive assisted living environment. Unlike the fast-paced demands of a hospital setting, assisted living offers a more stable and relaxed pace, allowing nurses to focus on long-term recovery, patient well-being, and quality of life.
Responsibilities
- Provide nursing care to residents in the assisted living facility under the supervision of the Registered Nurse
- Carry out assigned clinical tasks and responsibilities in accordance with facility standards
- Monitor residents’ conditions and report changes appropriately
- Support residents’ long-term care needs and overall quality of life
- Maintain accurate documentation and communication with the care team
- Work independently while ensuring high standards of patient care
Requirements
- Active New York State LPN license required
- CHHA experience preferred
- Strong ability to work independently with minimal supervision
- Excellent clinical skills
- Superior communication skills
- Basic knowledge of office applications and equipment, including Outlook, Word, printing, and scanning applications
Pay
$40/hr (plus Sign-On Bonus)
The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. When determining a team member’s base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

Monday–Friday, 11:30 AM–8:00 PM
