About the Role

As a Licensed Practical Nurse (LPN) at Accela Rehab and Care at Springfield, you will have the opportunity to deliver high-quality nursing care to residents in a warm and supportive environment. We are seeking enthusiastic and skilled LPNs to join our growing family, providing compassionate care to residents in our skilled nursing and long-term care setting.

Responsibilities

• Deliver high-quality nursing care to residents in accordance with care plans
• Collaborate with RNs, CNAs, and interdisciplinary teams to ensure optimal outcomes
• Monitor residents' health, administer medications, and document care accurately
• Provide emotional support and build meaningful relationships with residents and their families
• Maintain accurate and detailed records of resident care and interactions

Requirements

• Licensed Practical Nurse (LPN) in the state of Pennsylvania
• Preferred: Experience in a skilled nursing or long-term care setting
• Strong communication and teamwork skills
• A heart for elder care and a commitment to excellence
