About the role
Provides personalized, skilled nursing care in home settings, including medication administration, treatments, and specialized interventions such as trach/vent care and G-tube. Builds lasting patient relationships, monitors patient status, and educates patients and families for long-term success.
This role emphasizes compassionate, one-on-one care, fostering meaningful patient relationships in a supportive environment, with an emphasis on clinical excellence and personal flexibility.
Responsibilities
- Deliver direct patient care across pediatric, adult, or geriatric cases
- Coordinate with care teams and monitor patient health
- Provide education to patients and families
- Manage diverse case types including private duty, school nursing, and high/low-acuity pediatric cases
Qualifications
- Current LPN license and CPR certification
- Preferably 6 months to 1 year of nursing experience
- Strong clinical, communication, and interpersonal skills
- Reliable transportation for home visits
- New graduates are welcomed, with training and mentorship provided
Pay
