Provide direct patient care and support the nursing team in a Skilled Nursing Facility. Deliver compassionate care to residents, assist with daily living activities, administer medications, monitor vital signs, and collaborate with the healthcare team to ensure high-quality care while maintaining compliance with regulatory requirements.

Responsibilities

  • Provide direct, hands-on nursing care to residents, including administering medications, dressing changes, and treatments as prescribed by physicians.
  • Monitor and document residents' vital signs (blood pressure, pulse, temperature, etc.), noting any changes in condition and reporting to the Charge Nurse or Director of Nursing (DON).
  • Assist residents with daily activities such as bathing, dressing, grooming, and eating, as needed.
  • Observe and report any physical, mental, or emotional changes in residents’ conditions to the nursing supervisor.
  • Help implement individualized care plans in collaboration with the nursing and medical teams.
  • Assist with admission, transfer, and discharge procedures for residents, ensuring accurate documentation.
  • Administer medications as prescribed by physicians and in accordance with facility protocols.
  • Maintain accurate and timely documentation of medication administration and any changes in the residents' conditions.
  • Ensure proper storage and handling of medications, including controlled substances, following all regulatory guidelines.
  • Communicate effectively with residents, families, and other healthcare providers to ensure coordinated care.
  • Act as a liaison between the nursing team and other departments, ensuring that all aspects of care are being met.
  • Report changes in residents’ health conditions to the Charge Nurse and assist in providing follow-up care as needed.
  • Follow all facility policies, state regulations, and federal guidelines to ensure compliance with healthcare standards.
  • Participate in care planning meetings and assist in the development of care plans to improve residents' outcomes.
  • Participate in facility audits, quality improvement initiatives, and resident satisfaction surveys.
  • Maintain current licensure and stay up to date with best practices in nursing care through continuing education and training opportunities.
  • Assist in the training of nursing assistants (CNAs) and new nursing staff as needed.
  • Participate in team meetings, in-service training, and staff development opportunities.
  • Adhere to infection control protocols to ensure the safety of residents and staff.
  • Assist in maintaining a clean and safe environment for residents, staff, and visitors.
  • Report safety hazards or concerns promptly to the Charge Nurse or DON.

Requirements

  • Valid and current LPN license in the state of Oregon.
  • CPR and First Aid certification required (or must be obtained upon hire).
  • Prior experience in a skilled nursing or long-term care setting preferred but not required.
  • Experience with PCC is a plus.
  • Strong communication and interpersonal skills with the ability to interact effectively with residents, families, and healthcare team members.
  • Ability to work in a fast-paced environment, managing multiple tasks with attention to detail.
  • Compassionate, patient, and empathetic toward residents and their families.
  • Basic computer skills for documentation and communication.

Physical Requirements

  • Ability to stand, walk, and sit for extended periods of time.
  • Ability to lift or move residents, equipment, or supplies (up to 50 lbs).
  • Ability to respond to medical emergencies quickly and effectively.
