Patient Care
- Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
- Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
- Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.
Responsibilities
- Communicate effectively with residents, families, and the healthcare team.
- Maintain clear and accurate documentation of patient care.
- Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
- Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
- Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.
Qualifications
- Licensure: Valid LPN license in Nebraska.
- Education: Graduation from an accredited LPN program.
- Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.
- Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
- Communications: Excellent verbal and written communication skills.
- Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.
- Physical Requirements: Ability to lift and move patients, perform physical assessments, and stand for extended periods.
