About the role

Venture Community Services is seeking a Residential Licensed Practical Nurse (LPN) to provide care to a small group of individuals in a Residential home setting. The position offers an excellent benefits package, values work-life balance, and promotes a culture of inclusion and equity.

Responsibilities

• Assessments specific to each person’s health care needs.
• Assist in creating Problem/Needs list for each person in the residence with the nursing administrative team.
• Review nursing Problem/Needs list bi-monthly, after each hospitalization or as needed.
• Administer medications as assigned.
• Absorb and provide nursing care during on-duty hours.
• Follow plans of care for each person in the residence and facilitate health education and training of the direct care professionals.
• Physically assist individuals in transfer and carries, both ambulatory and non-ambulatory.
• Physically assist persons in bathing and personal hygiene tasks which may include lifts and physical support to persons with body weights of 150 - 250 pounds.
• Educate and assist staff and individuals with meal preparation, nutrition, and dietary needs.
• Handle related job duties in compliance with agency’s Policy and Procedure Manual.
• Identify safety risks and possible alternatives and advocate for the rights of all individuals.
• Maintain confidentiality in keeping with HIPPA regulations.
• Communicate any discharge instructions, treatments or medication changes to the RHM and all staff.
• Work with Home Health nursing if applicable.

Requirements

• Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
• At least one year of full time working experience as an LPN.
• Must be 21+ years old, have a valid driver’s license, daily use of a car on site and be available by telephone for daily contact.
