About the role

Venture Community Services is seeking a Residential Licensed Practical Nurse (LPN) to provide care to individuals with disabilities in a residential setting. The position involves assessing health care needs, administering medications, assisting with medical appointments, and supporting daily living activities.

Responsibilities

• Assessing health care needs for each individual
• Creating and reviewing problem/needs lists with the nursing administrative team
• Administering medications as assigned
• Aiding in health care provider visits
• Writing daily shift progress notes and weekly nursing assessments
• Notifying healthcare providers and the director of nursing of any medical changes or concerns
• Communicating discharge instructions, treatments, or medication changes to the Residential House Manager and staff
• Physically assisting individuals with transfers, carries, bathing, and personal hygiene tasks
• Educating and assisting staff and individuals with meal preparation, nutrition, and dietary needs
• Maintaining confidentiality and following agency policies and procedures
• Ensuring the program meets departmental and regulatory standards
• Participating in staff meetings and training sessions
• Providing recommendations for improving systems and processes

Requirements

• Current and valid registration as a professional nurse in Massachusetts
• At least one year of full-time working experience as an LPN
• Age 21 or older
• Valid driver's license and availability to use a car on site
• Ability to perform physical tasks such as lifting and carrying
