Long Term Care Functional Screener

About the role

A compassionate Long-Term Care Functional Screener is responsible for assessing eligibility for publicly funded long-term care services within the IRIS Program. The role involves building trusting relationships with participants, administering and documenting LTC Functional Screens, and guiding individuals to maintain funding eligibility. Collaboration with team members and community partners, participation in meetings, and preparing reports are essential duties. The role requires ensuring compliance with documentation standards and strengthening community connections to support participants.

Qualifications & Skills

  • Bachelor’s degree in social work, psychology, human services, nursing, or related fields
  • At least 1 year of experience with seniors or individuals with disabilities
  • Valid LTC Functional Screen Certification or willingness to obtain
  • Bilingual skills a plus (Spanish, Hmong, Arabic, Russian, etc.)
  • Strong organizational, planning, and communication skills
  • Comfortable with smartphones, tablets, and computer systems
  • Knowledge of human behavior, social dynamics, and community resources
  • Ability to work independently and collaboratively

Key Responsibilities

  • Conduct LTC Functional Screens and renewals
  • Provide guidance on maintaining funding eligibility
  • Develop trusting relationships with participants
  • Coordinate documentation and ensure regulatory compliance
  • Participate in team meetings and community outreach
  • Prepare reports and testify at hearings when needed

Additional Requirements

  • Valid driver’s license, reliable vehicle, satisfactory driving record
  • Willingness to travel frequently
  • Passing a background check
