• Aids with patient intake and scheduling support
• Coordinates insurance and financial assistance
• Arranges transportation and makes community resource referrals
• Plans discharges and transitions
• Advocates for patients and communicates with patients, families, and the dialysis care team
• Documents interactions and communications
• Floated to sister facilities

Requirements

• ND LMSW or LCSW
• 1+ years of experience in specialty

• Strong communication skills
• Ability to manage multiple tasks simultaneously
• Knowledge of insurance and financial assistance systems

Pay

$1600.00 - $2400.00 per week.
