Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.

Requirements

• Missouri LMSW license
• BLS certification
• Minimum of 2 years recent case-management experience in an acute hospital setting
• Acute-hospital experience in the specialty within the past year with no more than a 90-day break

Schedule

• 13-week assignment
• Shift: 8:00 AM – 5:00 PM
• Hours per week: 40
