Licensed Vocational Nurse - Home Health

About the role

We are seeking a Licensed Vocational Nurse (LVN) to provide skilled home health care to homebound patients in various locations.

Responsibilities

• Deliver nursing care as directed by registered nurses and physicians, following agency policies.
• Educate patients on self-care and rehabilitative measures, promoting health maintenance.
• Assist in care planning and communicate with healthcare professionals to ensure continuity of care.
• Document progress and follow care plans, prepare equipment, and assist with specialized procedures.
• Attend care conferences and maintain current knowledge of nursing trends.

Requirements

• Valid LVN license and driver’s license.
• Active BLS/ACLS/PALS certification.
• Reliable transportation and proof of auto insurance.
• Experience in home health care, ability to work independently, and proficiency with electronic health records.
• Physical and mental stamina, flexibility, and ability to remain calm under pressure.

Schedule

Flexible scheduling with a minimum of two days per week, including weekends and holidays. Work involves traveling to patients’ homes, with no remote work option.

Pay

Competitive pay per visit.
