About the Role
Provide direct patient care to pediatric, adolescent, adult, and geriatric patients in a home health setting.
Responsibilities
- Perform technical nursing functions under RN supervision
- Document patient progress
- Administer medications and treatments
- Maintain a safe environment for patients
- Identify abnormalities in patient condition
- Re-evaluate nursing needs
- Counsel patients and families
- Coordinate services with the care team
Requirements
- Graduate of an accredited nursing program
- Current LPN/LVN license
- At least one year of experience
- Valid CPR certification
- Valid driver’s license
- Ability to travel
