Home Health Licensed Vocational Nurse

Responsibilities

• Provide skilled nursing care in patients' homes, managing high-acuity cases including ventilators and tracheostomies
• Perform frequent assessments, administer medications, and implement individualized care plans under RN and physician supervision
• Document patient information accurately in the EMR system
• Educate families on care procedures and safety measures, promoting infection control

Qualifications & Skills

• Licensed Vocational Nurse (LVN) certification
• Experience with complex medical conditions and high-acuity nursing care
• Strong assessment, medication administration, and documentation skills
• Ability to educate and support families effectively
