Home Health Licensed Vocational Nurse

Role: In Home Healthcare LVN Responsibilities Provide high-acuity nursing care to pediatric and adult patients in their homes.Conduct skilled assessments, administer medications and treatments, and manage ventilator and tracheostomy care.Monitor complex patient conditions closely and collaborate with RNs, therapists, and families to implement personalized care plans.Ensure patient safety, educate caregivers, and maintain detailed documentation of care provided. Qualifications & Skills Licensed Vocational Nurse (LVN) license.Experience in home healthcare, pediatric and adult patient care preferred.Strong assessment, medication administration, and patient monitoring skills.Ability to work collaboratively with multidisciplinary teams and families.Excellent communication, attention to detail, and compassionate patient care approach.
