Licensed Social Worker- Long-term Care

Essential Duties

• Manage and direct the Social Services Department
• Identify and provide for residents' social, emotional, and psychological needs
• Accurately assess, document, and provide interventions to meet psychosocial needs of all residents
• Develop care plans based on assessments
• Maintain a quality working relationship with the medical profession and other health-related facilities and organizations

Preferred Qualifications

• Two years of job-related experience, preferably in long-term care
• Bachelor’s degree in social work or related field preferred
• Experience with social work, preferably working directly with patients and their family members at a nursing facility
• Detail oriented, computer literate, and possesses excellent communication skills
