About the role
The Social Worker is responsible for providing support and guidance to residents in need. This includes conducting assessments, developing care plans, and coordinating services.
Responsibilities
- Conducting assessments to identify needs and develop care plans
- Counseling and supporting residents in their personal and social development
- Coordinating services such as medical appointments, therapy sessions, and community events
- Maintaining accurate records and reporting progress to supervisors
Requirements
- Bachelor’s degree in Social Work or related field
- Valid driver’s license and access to reliable transportation
- Experience working with individuals with disabilities or mental health challenges
- Excellent communication and interpersonal skills
Qualifications
- Knowledge of community resources and service providers
- Ability to work independently and as part of a team
- Strong organizational and time management skills
