Social Worker Long Term Care

About the role

Ascension seeks a Comprehensive Psychosocial Assessment Specialist at Franciscan Place in Brookfield, WI. This position is part of the Long Term Care & Skilled Nursing department and works days, full-time.

Responsibilities

• Interview residents and families to evaluate social, emotional, and financial needs
• Meticulously document case activity to support individualized care planning
• Closely collaborate with clinical managers, families, and external healthcare agencies for full alignment and regulatory compliance
• Secure essential community assistance, healthcare benefits, and eldercare resources through targeted referrals and advocacy
• Proactively identify and address environmental safety hazards or emergencies to maintain a secure living environment
• Accurately complete federally mandated MDS assessments (including Section Q) within required timelines to support resident psychosocial well-being and facility funding

Requirements

• Licensure / Certification / Registration: Master Social Worker, preferred
• Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required
• Work Experience: 3 years of experience preferred
