About the role

Ascension seeks a comprehensive psychosocial assessments specialist to join the Franciscan Place team in Brookfield, WI. This position focuses on interviewing residents and families to evaluate social, emotional, and financial needs, ensuring meticulous documentation for individualized care planning.

Responsibilities

• Interview residents and families to evaluate social, emotional, and financial needs
• Meticulously document all case activity to support individualized care planning
• Collaborate with clinical managers, families, and external healthcare agencies to ensure full alignment and regulatory compliance with resident treatment goals
• Navigate community resources for residents and families, including securing essential community assistance, healthcare benefits, and eldercare resources through targeted referrals and advocacy
• Proactively identify and address environmental safety hazards or emergency situations to maintain a secure, high-quality living environment for all residents
• Maintain strict CMS compliance by accurately completing MDS assessments (including Section Q) within required timelines to support resident psychosocial well-being and safeguard facility funding

Requirements

• Licensure / Certification / Registration: Master Social Worker, preferred
• Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required
• Work Experience: 3 years of experience preferred

Qualifications

Comprehensive knowledge of social work principles and practices, strong interpersonal and communication skills, ability to work independently and collaboratively, and proficiency in Microsoft Office are required.
