Licensed Social Worker - Home Care

About the role

Provides direct social work services to patients, extending services to family members in need of identified social worker services, offering psychosocial and environmental support. Makes available a full range of treatment modalities with individuals, families, and groups, based on social work assessment and diagnosis. Coordinates social work services with other departments and providers within Park Nicollet Health Services, including hospice, hospital and clinic-based social workers, and out-of-system providers. May provide clinical supervision to other Social Workers. Works collaboratively with physicians, nurse clinicians, medical/surgical specialists, and other Park Nicollet Health Services employees.

Responsibilities

• Print Daily Census and identify high readmission risk patients.
• Team with RN Care Manager in the morning to review patients and assigned work.
• Provide psychosocial assessments and diagnosis, which may include differential diagnosis, use of diagnostic tools, and application of treatment modalities for patients and families.
• Mobilize and develop individual, family, and community resources in patient care planning and discharge planning to optimize continued health potential.
• Complete, coordinate, and provide continuity of care during the discharge process, which may be a shared duty with the RN Care Manager.
• Complete chemical dependency and psychiatric assessments and placements.
• Provide outpatient programs and community resources (e.g., meals on wheels, alternative care grant, chemical dependency/mental health).
• Provide financial resources (e.g., referral to financial counselors, med-eligible services (MES), MN Drug Card).
• Assist with Advance Care Planning (ACP) and Provider Orders for Life-Sustaining Treatment (POLST) completion as needed.
• Coordinate and attend patient care conferences as needed.
• Assess involvement in legal components of care, including child and adult protection reports, custody and adoption cases, commitment proceedings, Power of Attorney, and guardian issues.
• Conduct regularly scheduled and ongoing support groups for patients with complex diagnoses and their families.
• Participate in, promote, and direct fundraising activities dedicated to the community of patients.
• Attend monthly patient family council meetings, rotating throughout the social worker staff.
• Provide consultation and education to healthcare teams and selected healthcare systems regarding psychosocial issues for patients and their families.
• Perform related administrative tasks such as insurance authorization reports, statistics, and representing social work in the hospital, clinic, and community (e.g., Care Improvement Teams (CIT), Pre-operative joint class, Caring for Colleagues).

Requirements

• Master’s degree in social work.
• Minnesota (MN) Social Work license upon start date: Licensed Independent Social Worker (LISW) or Licensed Independent Clinical Social Worker (LICSW).
• Must have a vehicle, driver’s license, and current auto insurance.
• Must meet all criteria required under Park Nicollet’s Automobile Liability Guidelines.
• Basic computer/keyboarding skills.
• Ability to conduct individual and group therapy sessions.
• Ability to coordinate and facilitate group interactions.
• Knowledge of age-related growth and development principles necessary to provide appropriate service and ensure patient safety.
• Responsibility for travel arrangements required to meet the expectations of the position.

Preferred Qualifications

• MN licensure at the LICSW level.
• Previous Home Care experience.
• Advance Care Planning experience.
• Knowledge of word processing, spreadsheet, and presentation software.

Schedule

4 days per week, 8:00 AM – 4:30 PM, Monday through Friday.
