Social Worker - Home Care

About the role

Responsible for assessment and planning patient care in conjunction with other disciplines in home care. Provides social work service up to and including discharge for patients requiring those services. Assists clients in obtaining available community, State or Federal services.

Responsibilities

• Assesses and plans patient care in collaboration with other healthcare professionals.
• Provides social work services from admission to discharge.
• Affords assistance to clients in accessing available community, state, or federal services.

Requirements

• Bachelor's degree in Social Work.
• Current Kansas Social Worker License.
• Minimum 1 year Social Work experience in a Health Care setting.
• Auto liability insurance.
• Clean driving record.
• Current driver's license.
• Successfully passed KDADS criminal record check upon hire.
• Registered with and successfully passed the Family Care Safety Registry upon hire.

Qualifications

• Preferred: Current Clinical Social Worker licensure in state of practice.
• Continual Education: AHA BLS for Healthcare Providers.

• Strong organizational and problem-solving abilities.
• Ability to work independently and as part of a team.
