About the role

The Social Worker - Home Care position in Greensboro, NC is responsible for providing support to clients in their homes. This includes conducting assessments, developing care plans, and coordinating services to meet client needs.

Responsibilities

• Conduct home visits to assess client needs and provide necessary support
• Develop and implement care plans tailored to each client's unique situation
• Coordinate with other healthcare providers and community resources to ensure comprehensive care
• Communicate regularly with clients and families to monitor progress and adjust care plans as needed
• Ensure compliance with all relevant regulations and standards of care

Requirements

• Bachelor's degree in Social Work or related field
• Current license to practice Social Work in North Carolina
• At least 2 years of experience working with clients in a home care setting
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team

Qualifications

Pay

Salary range for this position is $45,000 - $55,000 annually, depending on experience and qualifications.

Schedule
