Responsibilities

• Provide emotional and logistical support to patients and their loved ones during their final days.
• Affirm patients' wishes regarding finances, funeral arrangements, and other matters.
• Navigate the grieving process for patients and their families.
• Grant end-of-life wishes and listen to patients' stories at their bedside.
• Counsel and intervene in crises related to terminal illness.
• Offer bereavement support and grief counseling.
• Educate patients and families about the emotional and psychological responses to illness progression.

Requirements

• Master's of Social Work from an accredited school, or equivalent experience.
• At least one year of social work experience in a healthcare setting.
• Knowledge of psychosocial issues affecting terminally ill patients and their families.
• Excellent listening and communication skills.
• Ability to work effectively as part of an interdisciplinary team.
• Commitment to patient welfare and compassionate care.
• Valid driver's license and reliable transportation.

Qualifications

• Licensure as a Social Worker (LSW) or equivalent in the state of employment.
• Thorough knowledge of community resources or the ability to acquire such knowledge.
• Experience working with terminally ill patients and their families.
• Empathy and compassion in dealing with seriously ill patients and their families.
