About the Role
Westside Nursing Home is a long-term care facility seeking a Licensed Social Worker to foster a climate, policies, and routines that enable residents to maximize their individuality, independence, and dignity. This role ensures residents achieve the highest practical level of physical, mental, and psychosocial well-being and quality of life. The position involves identifying psychosocial, mental, and emotional needs and providing or facilitating access to services that meet those needs, in accordance with the National Association of Social Workers (NASW) Code of Ethics and all applicable regulations.
Responsibilities
- Complete social history and psychosocial assessments for each resident to identify social, emotional, and psychological needs.
- Complete relevant parts of the Minimum Data Set (MDS) and utilize it to guide care plans.
- Maintain accurate and timely documentation in compliance with federal/state regulations, corporate policy, and center practices, including:
- Advanced Directives
- Minimum Data Set
- Social Service History and Assessment
- RAPs
- Care Plans
- Social Service Progress Notes
- Behavior Tracking
- Monitoring of cognitive and psychosocial changes
- PASARR and Discharge Planning documentation
- Psych services documentation
- Support residents' right to self-determination, document refusals of services, inform residents of therapeutic alternatives, and involve others as appropriate to educate residents about alternatives.
- Participate in developing a written, interdisciplinary plan of care for each resident that identifies psychosocial needs, goals, and appropriate social worker interventions.
Requirements
- Certified, licensed, or registered in the state of practice (required).
- Bachelor’s Degree in Social Work or a Bachelor’s Degree in a Human Services field (e.g., Sociology, Special Education, Rehabilitation Counseling, Psychology) from an accredited school (required).
- One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
