Licensed Social Worker

About the Role

The Licensed Social Worker (LSW) oversees the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current Federal, State, and local standards. The LSW works directly with discharge planners, family, or responsible parties to initiate tours of the facility and follow-up procedures.

Responsibilities

• Assist in the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current Federal, State, and local standards.
• Develop and manage initial contact with discharge planners, family, or responsible parties to initiate tours of the facility and follow-up procedures.
• Assist in developing and implementing policies and procedures for identifying the medically-related social and emotional needs of patients/residents.
• Develop and maintain a good working rapport with intra-department personnel, other departments within the facility, and outside community health, welfare, and social agencies to ensure social service programs meet patient/resident needs.
• Keep abreast of current federal and state regulations as well as professional standards.

Qualifications

• Must have a current Social Work license in good standing with the state (as required).
• Bachelor’s Degree in Social Work strongly preferred.
• Must have at least 3+ years of experience in a long-term care facility.
• Must be able to organize and prioritize tasks effectively.
• Skilled communicator, director, and motivator.
• Reliable transportation required.

Pay

Posted Salary Range: USD $35.00 - USD $37.00 /Hr.
